Resumo A fundação e edição da revista Penélope. Fazer e Desfazer a História foi um feito notável na carreira multifacetada, enérgica e aparentemente sem limites de António Hespanha. O título da revista encerrava a forma como a geração mais jovem de historiadores procurou expressar-se e sobretudo as novas direcções tomadas no campo da historiografia portuguesa após o 25 de Abril de 1974.

Abstract The founding and editing of the journal Penélope. Fazer e Desfazer a História was a notable achievement in António Hespanha’s multifaceted, energetic and seemingly limitless career. The title of the journal encapsulated the way in which the younger generation of historians sought to express themselves and, above all, the new directions that were taken in the field of Portuguese historiography following the revolution of April 25, 1974.

Tendo por base a lista das teses defendidas nas universidades portuguesas nos últimos oito anos, faz-se uma breve apreciação sobre os desenvolvimentos recentes no campo de Estudos de História Moderna em Portugal, destacando-se as suas tendências atuais e os seus problemas duradouros.

Based on the list of the theses defended in Portuguese universities in the last eight years, a brief assessment is made of the recent developments in the field of Early Modern History Studies in Portugal, highlighting its recent trends and persistent problems.

RESUMO OBJETIVO Decisões de limitação terapêutica (DLT) e de não reanimação (DNR) são difíceis, mas importantes, visando melhorar a qualidade de vida dos doentes e minimizar distanásia. O objetivo deste estudo foi avaliar a abordagem das DNR e DLT, as características dos doentes e a documentação dessas decisões numa Unidade de Cuidados Intensivos Polivalente (Ucip) em Portugal. MÉTODOS Análise retrospectiva dos dados referentes a DLT e DNR, recolhidos a partir de formulários previamente elaborados e complementados por consulta de processo clínico. RESULTADOS Um total de 1.602 doentes foi internado na Ucip entre 2011 e 2016. DNR foi documentada em 127 casos (7,9%). Doentes com DLT eram mais velhos e tinham um Simplified Acute Physiology Score II mais elevado. O diagnóstico mais frequente que precedeu essas decisões foi sepse (52,0%, n=66); A razão mais comum para limitar o tratamento foi mau prognóstico da doença aguda. Dos doentes nos quais a DNR foi implementada, 117 (92,1%) morreram na Ucip (40,1% do total de óbitos na Ucip) e a mortalidade hospitalar foi de 100%. Os intervenientes nessas decisões, bem como os tipos de tratamento retirados, não foram rotineiramente registrados. CONCLUSÃO As DLT e DNR foram relativamente comuns, em consonância com outros estudos do sul da Europa, mas atrás dos centros do norte da Europa e da América do Norte. Os doentes com essas limitações eram mais velhos e mais gravemente doentes. A documentação dessas decisões deve ser clara e detalhada, seja em formulários específicos, seja em registros clínicos informatizados. Há espaço para melhorias nessa área.

SUMMARY OBJECTIVE Treatment limitation, as well as do-not-resuscitate (DNR) directives, are difficult but important to improve patients’ quality of life and minimize dysthanasia. We aimed to study the approach to withholding, withdrawal, and DNR decisions, patients’ characteristics, and process documentation in a general Intensive Care Unit (ICU) in Portugal. METHODS A retrospective analysis of data regarding the limitation of treatment decisions collected from previously-designed forms and complemented by medical record consultation. RESULTS A total of 1602 patients were admitted to the ICU between 2011 and 2016. DNR decisions were documented in 127 cases (7.9%). Patients with treatment limitations were older and had higher Simplified Acute Physiology Score II. The most frequent diagnosis preceding these decisions was sepsis (52.0%, n = 66); the most common main reason for limiting treatment was a poor prognosis of acute illness. Of the patients to whom a DNR was implemented, 117 (92.1%) died in the ICU (40.1% of the total number of ICU deaths), and hospital mortality was 100%. Participants in these decisions, as well as types of treatment withdrawn and their respective timings, were not registered in medical records. CONCLUSION Treatment limitation and DNR decisions were relatively common, in line with other Southern European studies, but behind Northern European and North American centers. Patients with these limitations were older and more severely ill than patients without such decisions. Documentation of these processes should be clear and detailed, either in specific forms or computerized clinical records; there is room for improvement in this area.

Exactamente porque alicerçada com solidez no conhecimento de matrizes institucionais e sociais da realidade trans-continental portuguesa, os trabalhos de Romero Magalhães fornecem muitas e relevantes contribuições sobre matérias mais específicas. Muitas vezes, existem sugestões relativamente secundárias na economia global de uma obra que podem ser muito marcantes para quem as lê. Gostaria de sublinhar três, que influenciaram de forma decisiva a minha maneira de entender a história portuguesa e toda a minha produção historiográfica.

Precisely because they were grounded on a solid knowledge of institutional and social matrices of the Portuguese trans-continental reality, Romero Magalhães's works provide many relevant contributions on specific matters. Often there are less prominent ideas in the overall economy of a work that can be very striking to those who read them. I would like to emphasize three, which decisively influenced my understanding of Portuguese history and all my historiographical output.

Este artigo apresenta e discute duas Memórias de autoria de Luís Pinto de Sousa Coutinho (1735-1804), 1.º Visconde de Balsemão, que foram utilizadas como fonte para a elaboração da edição da Histoire des deux Indes, de 1780, no que diz respeito ao Brasil. As Memórias constituem um retrato singular e excecional do império português - as mais precisas e informadas que se conhecem para aquela data -, permitindo um substantivo acrescentamento do texto do Abade Raynal e, em particular, uma reorientação do mesmo, da qual resultou numa visão pouco crítica e até apologética da administração portuguesa e da escravidão no Brasil.

This article addresses and discusses the Memoirs of Luís Pinto de Sousa Coutinho (1735-1804), 1st Viscount of Balsemão, which were used as sources for the production of the 1780 edition of the Histoire des deux Indes, in respect of Brazil. The Memoirs offer a singular and exceptional portrait of the Portuguese empire - in the most precise and well-informed manner known for the time -, and represent a substantive addition to Abbé Raynal’s text and, in particular, a reorientation of its rather uncritical and even apologetic view of the Portuguese colonial administration and of slavery in Brazil.

RESUMO INTRODUÇÃO Eventos neurológicos agudos resultam frequentemente em incapacidade grave que impede o doente de participar ativamente nas decisões do seu próprio tratamento. A sobrevida a cinco anos ronda os 40%; metade dos sobreviventes fica dependente de terceiros. Objetivo Avaliar a sintomatologia de doentes internados com acidente vascular cerebral (AVC), hemorragia subarcnoideia (HSA) ou subdural (HSD) e analisar a intervenção de uma Equipe Intra-Hospitalar de Suporte em Cuidados Paliativos (EIHSCP). MATERIAL E MÉTODOS Estudo retrospetivo observacional dos doentes com diagnóstico principal de evento neurológico agudo com pedido de colaboração à EIHSCP, num hospital terciário, durante cinco anos (2012-2016). RESULTADOS Avaliados 66 doentes, com média de idade de 83 anos. Destacam-se 41 AVC isquêmicos, 12 hemorrágicos, 12 HSD e 5 HSA. A média da demora entre internamento e pedido de colaboração à EIHSCP foi de 14 dias. Na primeira observação, a média na escala de coma de Glasgow foi de 6/15 e na Palliative Performance Scale (PPS) foi de 10%. Disfagia (96,8%) e broncorreia (48,4%) foram os sintomas mais frequentes. A maioria dos doentes (56/66) mantinha sonda nasogástrica (84,8%); 33 encontravam-se em monitorização cardiorrespiratória (50,0%); 24 estavam sob hipocoagulação (36,3%); 25 necessitavam de opioide e antimuscarínico que não estavam prescritos (37,9%); seis tinham tubo orotraqueal, que foi retirado. A mortalidade intra-hospitalar foi de 72,7% (n=48). DISCUSSÃO E CONCLUSÃO Destaca-se o estado debilitado dos doentes; em muitos casos, intervenções fúteis persistiam, mas várias foram submedicadas para o controle dos sintomas. Verificou-se um tempo de espera elevado até o pedido de colaboração. Pela elevada morbilidade associada a esses eventos, cuidados paliativos diferenciados deveriam ser oferecidos no tempo adequado.

SUMMARY INTRODUCTION Acute neurological illness often results in severe disability. Five-year life expectancy is around 40%; half the survivors become completely dependent on outside help. OBJECTIVE Evaluate the symptoms of patients admitted to a Hospital ward with a diagnosis of stroke, subarachnoid hemorrhage or subdural hematoma, and analyze the role of an In-Hospital Palliative Care Support Team. MATERIAL AND METHODS Retrospective, observational study with a sample consisting of all patients admitted with acute neurological illness and with a guidance request made to the In-Hospital Palliative Care Support Team of a tertiary Hospital, over 5 years (2012-2016). RESULTS A total of 66 patients were evaluated, with an age median of 83 years old. Amongst them, there were 41 ischaemic strokes, 12 intracranial bleedings, 12 subdural hematomas, and 5 subarachnoid hemorrhages. The median of delay between admission and guidance request was 14 days. On the first evaluation by the team, the GCS score median was 6/15 and the Palliative Performance Scale (PPS) median 10%. Dysphagia (96.8%) and bronchorrhea (48.4%) were the most prevalent symptoms. A total of 56 patients had a feeding tube (84.8%), 33 had vital sign monitoring (50.0%), 24 were hypocoagulated (36.3%), 25 lacked opioid or anti-muscarinic therapy for symptom control (37,9%); 6 patients retained orotracheal intubation, which was removed. In-hospital mortality was 72.7% (n=48). DISCUSSION AND CONCLUSION Patients were severely debilitated, in many cases futile interventions persisted, yet several were under-medicated for symptom control. The delay between admission and collaboration request was high. Due to the high morbidity associated with acute neurological illness, palliative care should always be timely provided.

Introdução: As características únicas do doente com acidente vascular cerebral (AVC), pelo início súbito de compromisso funcional grave com incertezas no prognóstico, aumentam a necessidade de uma abordagem precoce por Cuidados Paliativos. O objetivo do estudo foi analisar o perfil de doentes com AVC a quem foi pedida colaboração da Equipa Intra-Hospitalar de Suporte em Cuidados Paliativos. Métodos: Estudo retrospetivo observacional com amostra constituída por todos os doentes com diagnóstico principal de AVC num hospital terciário a quem foi pedida colaboração à equipa num período de 5 anos (2012-2016). Resultados: Foram avaliados 48 doentes, com média de idade de 79,6 anos (± 13,1), 24 do sexo feminino. Destacam-se 41 AVC isquémicos, com National Institutes of Health Stroke Scale (NIHSS) médio à admissão de 18,1 (± 6,5) e 7 hemorragias intracerebrais. Os pedidos de observação foram para ajuda na organização de cuidados (89,6%) e tomada de decisões (70,8%), e para controlo sintomático: sintomas respiratórios em 58,3%, gastrointestinais (31,3%) e dor não-controlada (25,0%). Na primeira observação a Palliative Performance Scale foi 12,2 (± 8,1). A dispneia (52,6%) e broncorreia (50,0%) foram os sintomas mais frequentes e a pneumonite de aspiração a complicação mais prevalente (67,4%). Introduziu-se opióide em 45,7% e anti-muscarínico em 34,8%. Registou-se uma mortalidade intra-hospitalar de 72,9% (n = 35), com 71,4% das mortes a ocorrer nos primeiros 30 dias após o evento. Discussão e Conclusão: Pela complexidade e prevalência de sintomas não-controlados neste grupo de doentes, verifica-se que um AVC compromete significativamente o estado funcional, diminuindo a qualidade e esperança de vida. Estes são doentes que devem ter acesso inequívoco a Cuidados Paliativos.

Introduction: The unique characteristics of a stroke patient, due to the sudden onset of severe functional impairment, as well as prognostic uncertainty, increase the necessity of a timely Palliative Care approach. Our objective was to analyze the profile of stroke patients to whom In-Hospital Palliative Care and Support Team evaluation was requested. Methods: Retrospective observational study with a sample composed of all patients admitted with primary diagnosis of stroke in a tertiary hospital, with Team evaluation request, over a period of 5 years (2012-2016). Results: A total of 48 patients were evaluated, with a mean age of 79.6 years (± 13.1), 24 female. There were 41 ischemic strokes, with mean National Institutes of Health Stroke Scale (NIHSS) upon admission of 18.1 (± 6.5), and 7 cerebral hemorrhages. Evaluation requests were aimed at care organization (89.6%) and decision-making (70.8%), and for symptomatic control: respiratory (58.3%), gastrointestinal (31.3%) and uncontrolled pain (25.0%). At the time of the first evaluation, the mean Palliative Performance Scale 12.2 (± 8,1). Dyspnea (52.6%) and bronchorrhea (50.0%) were the most prevalent symptoms and aspiration pneumonitis the most common complication (67.4%). Opioid treatment was started in 45.7% and antimuscarinic in 34.8%. In-hospital mortality was 72.9% (n = 35), with 71.4% of deaths occurring on the first 30 days post-event. Discussion and Conclusion: Due to the complexity and prevalence of uncontrolled symptoms in this type of patients, it is clear that acute neurological events often result in severe functional impairment, reducing both quality of life and life expectancy. These are, therefore, unequivocally patients in need of swift access to Palliative Care

Introdução: A infecção por Staphylococcus aureus meticilino-resistente (MRSA) é uma das infecções associadas aos cuidados de saúde mais importantes. Em 2014 surgiu uma Norma da Direcção-Geral da Saúde dirigida a esta problemática. Este trabalho visa quantificar os doentes com indicação para pesquisa de portador de MRSA, analisar os critérios, implicações e possíveis estratégias. Material e Métodos: Recolha prospectiva de dados sobre os doentes admitidos num Serviço de Medicina em cinco semanas não consecutivas. Resultados: A amostra inclui 224 doentes, maioritariamente do género feminino, com média de idades de 76 anos. A maioria (60,3%) apresentou indicação para rastreio, realizado em 39,3%; 28,3% foram positivos. Os critérios mais frequentes foram antibioterapia prévia (37,1%), internamento recente (32,1%) e transferência de uma instituição hospitalar (16,1%). Discussão: Salienta-se a faixa etária avançada da população estudada, sendo que o factor idade parece influenciar a probabilidade de indicação para rastreio. A maioria dos doentes apresentou indicação para a pesquisa de portador (60,3%), o que constitui uma mudança de práticas de grande envergadura. Assume grande relevância o elevado peso da antibioterapia nesta amostra (37,1%). Numa maioria de doentes com indicação, o rastreio não foi realizado, sendo necessário sensibilizar a equipa médica e de enfermagem. Conclusão: É importante exigir boas práticas dos profissionais de saúde para a implementação de novas regras e garantir condições de funcionamento nos serviços para que as mesmas possam ser aplicadas. Torna-se crucial que as estratégias não sejam implementadas isoladamente. O cidadão deve cumprir a sua parte, nomeadamente evitando a toma de antibióticos injustificada.

Introduction: The infection by Staphylococcus aureus methicillin-resistant (MRSA) is one of the most important healthcare-associated infections. In 2014 there was a rule from the National Directorate of Health addressed to this problem. This study aims to quantify the patients with indication to search for MRSA colonization, examine the criteria, implications and possible strategies. Materials and Methods: Prospective data about patients admitted to an Internal Medicine Department in five non-consecutive weeks. Results: The sample includes 224 patients, mostly female, with a mean age of 76 years old. The majority (60.3%) presented indication for screening, which was conducted in 39.3% of the patients; 28.3% were positive. The most common criteria presented were prior antibiotic therapy (37.1%), recent hospitalization (32.1%) and transfer from a hospital institution (16.1%). Discussion: The age factor seems to influence the probability of indication for screening. Most patients showed indication to search for MRSA colonization (60.3%), which means a huge need for practices change. The high level of antibiotic therapy assumes great relevance in this sample (37.1%). In most of the patients, the screening was not done and it is necessary to sensitize the medical and nursing team. Conclusion: It is important to require good practices from health professionals for the implementation of new rules and ensure operating conditions in the services in order they can be applied. It is crucial that the strategies are not implemented alone. The patient must fulfill his part by not taking unjustified antibiotics.

Resumo Este artigo compara e analisa a parte sobre o Brasil na Histoire philosophique des établissements et du commerce des Européens dans les deux Indes, de Guillaume Thomas François Raynal, nas edições de 1770, 1774 e 1780. Pretende-se confrontar as ideias difundidas pela historiografia corrente sobre esta que foi a obra setecentista mais lida à época, com os juízos que, nas suas várias edições, vão surgindo acerca da América portuguesa. Para efeito, recortou-se alguns temas: primeiramente, os habitantes e os povoadores desse Novo Mundo, isto é, ameríndios, depois, negros e portugueses criolos - colonos nascidos no Brasil de ascendência portuguesa -, e, por fim, a administração, destacando-se o período pombalino e o Diretório dos Índios. Contra as opiniões mais difundidas, que apresentam a obra como uma "máquina de guerra" disparada contra a opressão e o colonialismo, o discurso de Raynal, particularmente na última edição (1780), menos coerente mas com uma maior riqueza de informações e novos juízos de valor, revela-se surpreendentemente benevolente em relação ao Brasil colonial. Ao contrário dos repetidos lugares comuns, os tópicos da miscigenação dos colonos, da libertação dos índios e da alforria dos escravos, explicação para a ausência de revoltas, antecipam os temas do luso-tropicalismo freyriano.

Abstract This article compares and analyzes the sections on Brazil in the 1770, 1774 and 1780 editions of Histoire philosophique des établissements et du commerce des Européens dans les deux Indes, by Guillaume Thomas François Raynal. The aim is to contrast the ideas disseminated by the current historiography about this eighteenth-century work, which was the most widely read of its time, with the judgments about Portuguese America that emerge in those editions. In effect, some issues stand out: first, the inhabitants and settlers of this new world, that is, Amerindians, then Blacks and Creole Portuguese - Brazilian-born settlers of Portuguese descent - and finally, its administration, highlighting the Pombaline period and the Indian Directorate. Contradicting the prevalent opinions that see this work as a "war machine" fired at oppression and colonialism, Raynal's discourse, which is less consistent but contains a greater wealth of information and new value judgments, particularly in the last edition (1780), proves to be surprisingly benevolent towards colonial Brazil. Unlike repeated commonplace observations, the topics of the miscegenation of the settlers, the liberation of the Amerindians and the manumission of slaves - an explanation for the absence of revolts - foreshadow the themes of Freyrian Lusotropicalism.

Kenneth Waltz resistiu a descrever o mundo do pós-Guerra Fria como unipolar. Esta relutância resulta de duas características do realismo estruturalista de Waltz: a sua visão naturalista dos equilíbrios de poder e o seu enfoque nas relações entre grandes potências. Felizmente, o pensamento de Waltz sobre a revolução nuclear permite corrigir estas limitações da sua teoria estrutural e iluminar dois aspectos centrais da unipolaridade americana: a sua potencial durabilidade num mundo nuclearizado e o seu potencial para guerras preventivas contra a proliferação nuclear. Em suma, o realismo estruturalista de Waltz, quando combinado com o seu pensamento sobre o mundo nuclear, produz um potente aparato analítico para melhor compreender o mundo unipolar do pós-Guerra Fria.

Kenneth Waltz resisted describing the post-Cold War world as unipolar. This reluctance results from two characteristics of his structural realism: its naturalistic view of systemic balances of power and its focus on relations among great powers. Yet, Waltz’s thinking on the nuclear revolution allows us to correct these limitations of his structural theory and highlight two central aspects of American unipolarity: its potential durability in a nuclear world and its potential for preventive counter-proliferation wars. In sum, Waltz’s structural realism, when combined with his thinking about a nuclear world, produces a powerful analytic apparatus to further our understanding of our post-Cold War unipolar world

A hepatite alcoólica aguda é uma condição de difícil diagnóstico, em relação com o seu pleomorfismo clínico, associado, por vezes, apenas a discretas alterações analíticas, muitas vezes presentes em doentes com história de consumo abusivo de bebidas alcoólicas. Os autores pretendem fazer uma revisão teórica dos conceitos mais atuais no que diz respeito a diagnóstico e terapêutica desta entidade potencialmente fatal.

Acute alcoholic hepatitis is an often misdiagnosed condition, which relates to its multiple clinical manifestations, in association with mild analytical changes, often present in patients with significant history of alcoholic consumption. The authors intend to review the current knowledge about the diagnosis and treatment of this potentially fatal disease.
